Special Circumstance #3:
Affidavit of ResidencyIF YOU LIVE WITH A HAZELWOOD RESIDENT (AND YOU ARE NOT THE LEASEHOLDER OR HOMEOWNER) ON A VOLUNTARY BASIS (FOR REASONS OTHER THAN THOSE THAT QUALIFY FOR THE MCKINNEY VENTO HOMELESS ASSISTANCE ACT PROGRAM), PLEASE REVIEW THE AFFIDAVIT OF RESIDENCY REQUIREMENTS BELOW. IF YOU HAVE ANY QUESTIONS REGARDING THIS PROCESS, PLEASE CONTACT THE OFFICE OF ENROLLMENT AT 314-953-5141 OR ENROLLMENT@HAZELWOODSCHOOLS.ORG. Affidavit of Residency Requirements
Please provide the following documents to satisfy the Affidavit of Residency process.
1. Completed, signed, and notarized Affidavit of Residency Form (Leaseholder or Homeowner must be present)
2. Signed Statement of Financial Responsibility
3. One item from List A and one item from List B in the name of the leaseholder or homeowner in which you reside:
List A documents
· Deed
· Mortgage statement dated within 30 days of the date of submission
· Unexpired lease/rental agreement signed by landlord and tenant and including the address, apartment number, and term of the agreement
· Real estate tax receipt dated within 1 year of the date of submission
List B documents
One utility bill dated within 30 days of the date of submission for verification. Utility bills should include customer name, service address, and billing date. Examples of utility bills we accept include:
· Electric bill
· Water bill
· Gas bill
· Sewer bill
· Trash bill,
· Internet bill
· Cable bill
4. Leaseholder or homeowner’s occupancy permit listing 1) the parent/guardian of the child/children being enrolled, OR 2) listing the children to be enrolled in the district
5. Leaseholder or homeowner’s government issued, unexpired i.d. (NOTE: The government issued, unexpired i.d. should be for the same individual listed on the proof of residency documents)
6. Parent’s government issued, unexpired i.d.
7. Birth certificate(s) for child/children you are enrolling
8. Two pieces of recent credit mail, dated within the last 30 days, for the address in which you currently reside. Credit mail includes: medical bills, paycheck stub, student loan statements, insurance statement, credit card
Special Circumstance #4:
Special Enrollment WaiverIF YOU ARE ENROLLING A CHILD OF WHICH YOU ARE NOT THE BIOLOGICAL PARENT, MILITARY POWER OF ATTORNEY, OR LEGAL COURT-APPOINTED GUARDIAN (AND THE CHILD OR YOUTH DOES NOT QUALIFY FOR THE MCKINNEY VENTO HOMELESS ASSISTANCE ACT PROGRAM), PLEASE REVIEW THE SPECIAL ENROLLMENT WAIVER PROCESS REQUIREMENTS BELOW. IF YOU HAVE ANY QUESTIONS REGARDING THIS PROCESS, PLEASE CONTACT THE OFFICE OF ENROLLMENT AT 314-953-5141 OR ENROLLMENT@HAZELWOODSCHOOLS.ORG. THE SPECIAL ENROLLMENT WAIVER PROCESS MUST BE COMPLETED IN-PERSON. THE ENROLLMENT TEAM WILL BE LOCATED AT THE HAZELWOOD ADMINISTRATION BUILDING (15955 NEW HALLS FERRY ROAD, ST. LOUIS MO 63031). SPECIAL ENROLLMENT WAIVER REQUIREMENTS
Please provide the following documents to satisfy the Special Enrollment Waiver process.
1. Completed and signed Special Enrollment Waiver Form
2. Signed Statement of Financial Responsibility
3. One or more of the supporting documents listed below (in the name of the enrolling parent/guardian):
a. 2022 or 2023 Tax Documents to include the name of the dependent child you are enrolling
b. Recent Social Security Benefit Statement to list you as the Representative Payee
c. Proof of health insurance (listing the child as your beneficiary)
d. State or Federal assistance benefit letter naming the child as a member of your household
4. Parent/Guardian’s government issued, unexpired i.d.
5. Birth certificate(s) for child/children in which you are applying for the Special Enrollment Waiver
*If the enrolling parent/guardian is not able to provide any of the aforementioned documents please obtain a written delegation from the biological parent or legal guardian of the child, explaining the hardship of the family and why the student currently resides with you. Upon review, the Director of Enrollment will determine if the “waiver of residency” will be extended and/or additional documentation will be accepted.
Special Circumstance #5:
Temporary or Administrative ApprovalIF YOU HAVE RECENTLY MOVED TO HAZELWOOD SCHOOL DISTRICT AS A LEASEHOLDER OR HOMEOWNER, BUT YOU ARE UNABLE TO FULLY SATISFY PROOF OF RESIDENCY, PLEASE CONTACT THE ENROLLMENT OFFICE AT 314-953-5141 OR ENROLLMENT@HAZELWOODSCHOOLS.ORG
