Online Enrollment (New and Returning Families)

  • New Students Online Registration & Enrollment (new students)

    1. If you are a new family to the Hazelwood School District and do NOT have a personal email address, please create an email. Online enrollment and registration require an active email address. 

    2. If you are not a lease/deed holder, you must receive an Affidavit to enroll in the Hazelwood School District. Please contact our Residency and Enrollment Department at 314-953-5041 or send an email to residency@hazelwoodschools.org to request an Affidavit.

    3. If you are a lease/deed holder who resides in the Hazelwood School District and know your school of attendance, please see the School and Programs list and contact your school directly to complete the enrollment and registration processes. Phone numbers and email addresses for each school can be found at the bottom of the homepage of each school website. Additional school email addresses can be found in the “About Us,” “Teacher,” “Staff,” or “Administration sections of school websites. Typically, secretaries or assistant principals is the best person to contact to complete the process. If you do not know your school of attendance, please contact our Residency and Enrollment Department at 314-953-5041 or email us at residency@hazelwoodschools.org.

    4. If your living arrangement does not fit in any of the categories above, please contact our Residency and Enrollment Department at 314-953-5041 or email us at residency@hazelwoodschools.org.

  • Returning students Online Registration (returning students)

    1. If you have a returning student(s) to the Hazelwood School District, please login to the Tyler SIS Parent Portal to complete online registration forms. If you forgot your password, please reset your password by clicking on the “Forgot your parent password” tab on the Tyler SIS Parent Portal login page.

    2. If you are a lease/deed holder and need to update your address, please see the School and Programs list to contact your child's school directly. Phone numbers and email addresses for each school can be found at the bottom of the homepage of each school website. Additional school email addresses can be found in the “About Us,” “Teacher,” “Staff,” or “Administration sections of school websites. Typically, secretaries or assistant principals are the best person to contact to complete the process.

    3. If you need to renew an Affidavit, please contact the Residency and Enrollment Department at 314-953-5041 or email us at residency@hazelwoodschools.org.

    4. If your living arrangement does not fit either of the categories above, please contact our Residency and Enrollment Department at 314-953-5041 or email us at residency@hazelwoodschools.org.

  • Adding Students Online Registration (adding students)

    If you already have a student(s) enrolled in the Hazelwood School District and would like to add a student please see the guidance below: 

    1. If you are a lease/deed holder and would like to add a student, please see the School and Programs list to contact your child's school of attendance directly.  Phone numbers and email addresses for each school can be found at the bottom of the homepage of each school website. Additional school email addresses can be found in the “About Us,” “Teacher,” “Staff,” or “Administration sections of school websites. Typically, secretaries or assistant principals are the best person to contact to complete the process.
