Dear Hazelwood School District Community,

The Saint Louis County Department of Public Health (DPH) has begun offering mandatory back-to-school immunizations for children ages 3 to 18 at no cost. Those who are eligible will also have the option of getting a COVID-19 vaccine (ages 12 and up). 

The immunization events will run from 8 a.m. to 4 p.m. Monday through Friday until Sept. 17. The events will be held at:

• John C. Murphy Health Center, 6121 North Hanley Road in Berkeley
• North Central Community Health Center, 4000 Jennings Station Road, in Pine Lawn
• South County Health Center, 4580 S. Lindbergh Blvd. in Sunset Hills

DPH will conduct a special back-to-school event from 9 a.m. to noon on Aug. 14 at the John C. Murphy Health Center. Additional opportunities will be available from 4 to 7 p.m. on Wednesday evenings at the South County Health Center. 

Students are asked to bring a copy of their shot records with them. Please note that the clinics will close and no immunizations/vaccinations will be offered on Labor Day, which is Sept. 6.
