Transcripts for past students can be obtained by filling out the transcript request form and emailing it to transcripts@hazelwoodschools.org. For current students, transcripts can be obtained by contacting your school counselor.

Students who have graduated from Hazelwood School District within the last three years should request their transcripts from the last school of attendance. 

You must provide a copy of your photo identification with each transcript request.
