Parents/caregivers, please complete daily monitoring of your student’s health using the Student Daily Health Screen below). This screening is a tool for parents/caregivers to assist HSD in keeping the district healthy. This is not intended for the diagnosis or treatment of disease or other conditions, including coronavirus (COVID-19), but rather to identify students with possible infectious illness before they arrive at school.
If your student answered no to all of the symptoms listed in the flow chart and have not had a known exposure to COVID-19, they can attend school in person.
If your student answered yes to any of the symptoms listed in the flow chart and the symptoms cannot be explained by another pre-existing illness or condition, or if they have been exposed to someone who has tested positive for COVID-19 or is having symptoms and is awaiting testing, they need to stay home from school.
