     Thursday 8:30 am to 4:30 pm

     Friday 8:00 am to 5:00 pm

    Required Documentation

    · Consent forms can be found at https://carestlhealth.org/services/school-based-health/

    · A referral will need to be completed by a member of the staff within the school district to ensure we know what concerns need to be addressed.

    Payment

    · Please provide your Medicaid or Insurance card at the time of your visit

    · If you do not have insurance, please speak to our School-Based Health Services clinic team about getting assistance with filling out a Medicaid application or finding other coverage options.
