Required Student Immunizations Due by August 23, 2021
Dear Hazelwood School District Parent/Guardian,
The state of Missouri requires all students to have up-to-date immunizations to attend school.
The district must receive the appropriate documentation of state-required immunizations for your student before enrollment/registration is complete for the 2021-2022 school year.
Please reach out to your child’s pediatrician or community clinic for proper immunizations or documentation needs to prevent any delay to school access. All exemption cards must be the original card and be on file with the school nurse.

To assist families with completing this requirement, we have vaccine clinics scheduled through July and August with CareSTL and Children’s Hospital for those who would like the opportunity to be vaccinated with our district partners. Students who have private insurance, underinsured, or are uninsured can receive vaccines.
Appointments for these clinics must be made in advance. To assist you with scheduling an appointment, please contact the district by calling 314-953-5032, 314-953-5141, or emailing us at firstname.lastname@example.org. CareSTL Health can also be reached directly at 314-882-1284 to book appointments. When booking an appointment, please include your name, your child's name, and your best phone contact information.
Additionally, immunizations and the COVID-19 vaccine can be secured through St. Louis County. See the attached flyer for details.
Parents should contact their schools to submit the proper immunization documents or send documentation to email@example.com. Please note that the COVID-19 vaccine is not currently a required immunization.
