1. Vendors requesting to do business with the Hazelwood School District are required to complete a VENDOR REGISTRATION packet. Please note that you will need a signed and dated W-9 (electronic copy) in order to complete the vendor registration package. Completing the registration does not guarantee future business with the Hazelwood School District.

The Hazelwood School District is soliciting proposals from qualified individuals, firms or organizations to provide Medical TPA/Network/Cost Containment Services and Pharmacy Benefit Management Services. Interested parties should contact the representative below no later than January 20, 2020, at 3:00 p.m. to provide an intent to quote.
